Information for Patients - Direct Purchase of Nexavir®

As a service to patients who have obtained a prescription from their physician and are seeking a pharmacy to have their prescription filled, Nexco Pharma is able to provide the following information for The Custom Prescription Shoppe (CPS), a mail-order pharmacy that maintains an inventory of Nexavir ® and is able to fill your subscription upon receipt of the following:

Patient should fax or email the following information to CPS:

• Patient Name, shipping address, and phone number to contact patient in the event of questions (email is optional)
• Patient Date of Birth
• Existing medical conditions and drug allergy information
• Physician's name, phone and fax
• Credit Card information – (VISA, MC, American Express, or Discovery) including Authorization Number on the card

Patients must have their physician
fax or phone prescription directly to CPS.

Patient provided copies cannot be accepted.

Upon receipt of the above, CPS will fill the prescription within 1-business day according to the following pricing schedule:
